Supportive Living – Community Inclusion

Our Supportive Living and Community Inclusion programs include Outreach Support, Miller Block Cluster Living, Employment, Community Based, and Skill Development programming, and support individuals under the care of Community Living British Columbia (CLBC) who are eligible for both the Developmental Disabilities and Personal Supports Initiative funding streams.

Our programs provide collaborative, one-to-one employment, community-based, and supported living skills support. Many of the individuals that we serve are living in a community setting – in apartments, low-cost housing, hotels, supportive living settings, or Home Shares for example. Our programs focus on personal routines, community awareness, and social maturity. The individual, their facilitator, and the Outreach Worker jointly negotiate goals. When appropriate, the input of family members and other community stakeholders is incorporated as much as possible.

Our Miller Block supported living apartment program was developed in 2005 as a response to the need for safe, affordable housing for developmentally disabled individuals at risk of homelessness. The building encompasses 12 suites for individuals with developmental disabilities who choose to live independently. Tenants at Miller Block are referred by CLBC, and develop an individualized plan of care in collaboration with their facilitator and the Miller Block Outreach worker. Outreach support is provided on an as-needed basis, and is available to tenants 8 hours per day, 7 days a week.

• To support individuals to live as independently as possible while ensuring opportunities to participate in community activities of their choosing
• To enhance and support quality of life
• To promote independence by providing life skills training through person-centered plans developed by the person served and their Outreach worker
• To increase inclusion in the community, neighbourhood, and within age-affiliated activities
• To provide or assist individuals in securing stable and affordable housing
